From the cruise ports (pier 3 & 4) walk westward along the water until you get to Plaza Darsenas.
Pan-American Pier is NOT within walking distance.
You can take Uber or Taxis to Plaza Darsenas.

End point

After the tour inside El Morro Castle, participants can stay in the fort to explore it on their own.

Steps away from El Morro Castle there is a traffic circle (roundabout) where ride share apps (Uber, Lyft, etc.) offer pick up and where there are taxis standing by.
